Activate the WP Security Question Plugin. Go to Settings → Security Questions to Setup the questions.
A list of default questions will appear. If you wish, you can also add your own questions by clicking on the Add more option at the bottom. As you scroll to the bottom of the page, you will find the options to enable the security questions on login screen, registration and forgot password screen.
Check the ones you need for your website and click on Save Setting to save the changes you made.
A piece of cake, isn’t it? That is all. After you save your settings, you will be greeted with a change in your login screen. The “register new user” dialog box will also have similar changes. Below the password field, you can find the security questions. You will be required to choose your security question and answer it.
The registered users on your site can visit their Profile → My security question to choose a security question for themselves and add an answer.
